Under-Eye Fat Repositioning in Seoul, Korea
Scarless eye bag surgery that moves your own under-eye fat into the tear-trough hollow instead of removing it — flattening the bag and filling the groove in one operation, performed by board-certified plastic surgeon Dr. Lee Young-geun in Gangnam.

What is under-eye fat repositioning?
Under-eye fat repositioning is a fat-preserving eye bag operation: the bulging lower-eyelid fat is released and transposed below the orbital rim to fill the tear-trough groove. It is done through a small incision inside the lower eyelid (transconjunctival), so there is no external scar and no stitches to remove. It takes about 40–60 minutes under local anesthesia with sedation, most people return to office work in 3–5 days, and the final contour settles at 3–6 months.
Why choose Sinsa L for fat repositioning
- Signature operation — under-eye fat repositioning is the procedure the clinic was built around.
- One surgeon, every step — Dr. Lee marks, operates and reviews every case himself.
- Septal reinforcement — after the fat is transposed, the weakened septum is re-supported to lower the chance of the bag returning.
- Awake symmetry check — under sedation you can open your eyes before closing, so the result is judged on a live face.
- International-friendly — English support, photo pre-consults and a short-stay plan with no stitches to remove.
Who is a good candidate?
- A bulge under the eye that looks worse in the afternoon or in overhead light
- A visible groove running from the inner corner toward the cheek
- Dark circles that fade when you gently lift the cheek (shadow type)
- Reasonably elastic lower-lid skin without heavy creasing
- Typically late twenties to fifties, wanting a natural, unoperated look
How fat repositioning is performed
1. Mapping the rim and the bag
Under magnification and in several gaze positions, the orbital rim, tear-trough ligament and true fat herniation are assessed — two similar-looking bags often need different amounts of transposition.
2. Transconjunctival access
A small incision inside the lower lid; skin, muscle and lash line are left untouched, so there is no external scar and the lid margin keeps its position.
3. Release and transposition
The ligament attachment along the rim is released and the fat is moved down as a flap with its own blood supply, then spread and secured inside the hollow.
4. Septal reinforcement and check
The septum is tightened to restore the barrier, then symmetry is confirmed with your eyes open before closing.
Not the right fit? Loose, crepey skin is better served by lower blepharoplasty, and pigment-type dark circles do not change with surgery. Dr. Lee will tell you which applies to you.
Under-Eye Fat Repositioning before & after
This patient had a fat bulge with a shadowed groove beneath it. The bulging fat was moved into the groove and the septum reinforced, so the bag flattened and the shadow faded without adding fillers.
Photos are taken with standardized distance, lighting and angle. Final contour is usually seen at 3–6 months.
Fat repositioning vs. fat removal vs. lower blepharoplasty
| Fat repositioning | Fat removal | Lower blepharoplasty | |
|---|---|---|---|
| Best for | Bulge plus tear-trough groove | Isolated bulge, firm skin, no groove | Bulge plus loose, wrinkled skin |
| Incision | Inside the eyelid, no scar | Inside the eyelid, no scar | Fine line under the lashes |
| The fat | Moved into the groove | Taken out | Repositioned or reduced; excess skin trimmed |
| Time | About 40–60 min | About 30–40 min | About 60–90 min |
| Stitches | None to remove | None to remove | Removed at day 5–7 |
Your treatment journey
-
Consultation
Examination of the bulge, the groove and skin quality; photo review and a clear yes/no on candidacy.
-
Pre-op preparation
Health review, medication guidance and instructions (no smoking; blood thinners adjusted as directed).
-
Surgery day
Local anesthesia with sedation, about 40–60 minutes, awake symmetry check, home the same day.
-
Review
Clinic check at day 3–5. No stitches to remove; most overseas patients fly home after this visit.
Recovery timeline
- Day 0: cold compresses and head elevation; mild pressure rather than pain
- Days 1–3: swelling peaks; any redness in the white of the eye is harmless and clears on its own
- Days 3–5: clinic review; most office workers return to work
- Week 2: most visible swelling has settled; light makeup is usually fine
- Weeks 4–6: contour looks natural to others; exercise resumes
- Months 3–6: final settling as the transposed fat integrates
Aftercare tips
- Cold compresses for the first 48 hours, then as advised
- Sleep with your head elevated for about one week
- Avoid heavy exercise, saunas and alcohol for about 2 weeks
- Use prescribed eye drops and avoid rubbing the eyes
Risks & considerations
Temporary swelling and bruising, subconjunctival redness, dry-eye sensation, mild asymmetry during healing, under- or over-correction and, rarely, the need for revision. The inside-the-lid approach avoids a skin scar but not surgical risk; Dr. Lee reviews each of these with you before you decide.

Frequently asked questions
How is fat repositioning different from fat removal?
Removal takes the bulging fat out. Repositioning moves the same fat down into the hollow, so the bulge flattens and the groove fills at the same time. When a real tear trough exists, removal alone can leave the area flat but still shadowed — or more hollow with age.
Will there be a visible scar?
No. The incision is made on the inner surface of the lower eyelid and heals without a mark on the skin. There are no stitches to remove.
Does it fix dark circles?
It fixes shadow-type dark circles caused by the bulge-and-groove contour. It does not change pigment from melanin or thin skin showing veins; the consultation will tell you which type you have.
How long does the result last?
The repositioned fat is living tissue with its own blood supply and stays where it is placed. Ageing continues elsewhere, but the corrected bag does not usually return in the same form, especially when the septum has been reinforced.
Is general anesthesia needed?
No. It is done under local anesthesia with sedation, so you can open your eyes for the symmetry check and go home the same day.
How long should international patients stay in Seoul?
Most plan 5–7 days: consultation and surgery on day 1–2, a review at day 3–5, then the flight home.
